HIGH RISK

Description: Amitriptyline and perphenazine are used together in an oral preparation to treat major depression and anxiety and agitation. Amitriptyline is a tricyclic antidepressant agent used to treat depression. Perphenazine is an antipsychotic agent used for anxiety and agitation. Doses of each ingredient should be titrated and individualized. If the optimal dose corresponds to the ratio contained in the combination drug, this product can be used for convenient dosing.

Available as: EtrafonTM or TriavilTM tablets: amitriptyline 10 mg or 25 mg perphenazine 2 mg EtrafonTM -A tablets or TriavilTM tablets: amitriptyline 10 mg perphenazine 4 mg EtrafonTM -Forte tablets or TriavilTM tablets: amitriptyline 25 mg perphenazine 4 mg TriavilTM tablets: amitriptyline 50 mg perphenazine 4 mg

Dosage: Adults: 1 tablet PO 3-4 times per day of any combination, depending on requirement and patient response. The daily dosage should not exceed 200 mg amitriptyline or 32 mg perphenazine.

Children >12 years: Initial dosage is lower than adult dosage. Regimen should be individualized.

Children <12 years: Safety and efficacy have not been established.
