MODERATE RISK

Description: Molindone is an oral dihydroindolone antipsychotic agent used for the symptomatic management of psychotic disorders. It has high extrapyramidal effects, low sedative effects, and low anticholinergic effects compared with other phenothiazines. This drug was approved by the FDA in 1974.

Contraindications: Severe cardiovascular disorders. Drug Interactions: CNS depressants (e.g., opiates, alcohol): Can lead to excessive sedation or CNS depression. Anticonvulsants: Phenothiazines can lower the seizure threshold, so adjustment of anticonvulsant therapy may be necessary.

Adverse Reactions: Extrapyramidal effects, tardive dyskinesia, xerostomia, constipation, hypotension, sinus tachycardia, syncope.

Dosage: Adults: Initially, 50-75 mg PO tid-qid, increased if necessary in 3-4 days to 100 mg/day. Maintenance dosage varies from 5 mg PO tid or qid to a maximum of 225 mg/day for severe cases.
