I am not surprised you got a low response rate.  It would be
MUCH easier to be able to fill it out and submit it on-line.

Lacking that, you could get a newer version of Adobe Acrobat
that includes forms you can fill out.  You would still have
to mail it in, but you could at least use the computer to
fill it out and then print it out with the answers.
