How do they know I'm lacking dopamine when there is no blood test to
measure how much I have? (and I don't want to wait til I die to find out)

I am not sure dopamine finds itself in blood. It is a neurotransmitter that makes neurones connect but i am not sure it finds itself in the blood.

I don't think the disease process is understood at all, even  though they
say it is, but then what do I know?

They are beginning to unravel it but just the lack or uneven release of dopamine may cause very different smptoms.
